Autodigestion β digestive enzymes activate inside the pancreas instead of the duodenum, so the pancreas digests itself. Epigastric/LUQ pain that radiates to the back, sky-high lipase, and a gut that must be put to complete rest (NPO) while IV fluids and pain control lead the way.

CAUSE
STEP 1 Β· AUTODIGESTION
One organ, two jobs β when the digestive job turns on itself, the endocrine job (glucose control) gets dragged down with it.
π₯ The pancreas has two completely different jobs
Exocrine = digestive enzymes into the duodenum. Endocrine = hormones into the blood. Pancreatitis is an exocrine disease that drags the endocrine side down with it.
π§ βThe chef poisons the kitchen.β Enzymes are supposed to cook (digest) food in the duodenum β in pancreatitis they turn on early, inside the pancreas itself.

π§ βBooze & Stones.β If an NCLEX stem doesn't name a drug or CF, the answer is almost always alcohol or a gallstone.

π§ Acute = attack. Chronic = aftermath. Chronic disease is why students learn pancrelipase β the scarred gland can't make its own enzymes anymore.

CLUES
STEP 2 Β· SPOT IT
Pain location, two hemorrhagic skin signs, and the one lab that beats the others.
πͺ Pain β epigastric / LUQ radiating straight to the back
Sudden, severe, boring pain β worse lying flat or after a fatty meal/alcohol; classically eased by sitting up & leaning forward (fetal/knee-chest position takes pressure off the retroperitoneal pancreas).
π§ βLUQ, then out the back door.β If a stem says epigastric pain boring through to the back, think pancreas first.
π¨ Cullen's sign vs Grey Turner's sign β hemorrhagic pancreatitis
Blue-gray bruising = retroperitoneal bleeding tracking through tissue planes β hemorrhagic/necrotizing pancreatitis, a severe complication.
π§ βCullen circles the belly button, Turner takes the flanks.β Both = internal bleeding, not a bruise from trauma.
π§ͺ Labs: lipase beats amylase
Lipase is more specific to the pancreas & stays elevated longer β trust lipase over amylase.
π§ βLIPase LINGERS.β Both rhyme with staying power β lipase is the one you can still catch days later.
NCLEX TIP Fluids + pain control together are the top priority intervention for acute pancreatitis β not just one alone.
π§ βNPO, IV, and PAIN-O.β Nothing by mouth, fluids running, pain treated β in that order of urgency.
β Traditional teaching: avoid morphine
Older NCLEX-style teaching says avoid morphine β thought to spasm the sphincter of Oddi and worsen pain. Hydromorphone (Dilaudid) is the classic preferred choice.
π§ Practice varies by facility β for exam purposes, remember hydromorphone yes, morphine caution.
Acute phase: NPO β clear liquids β advance to low-fat, low-sugar diet as tolerated. Fat is the biggest enzyme trigger.

In chronic pancreatitis, the gland can no longer make its own enzymes β lifelong pancreatic enzyme replacement (pancrelipase) with every meal and snack β see NG-217 for full drug teaching.
π§ βLow fat keeps it flat.β A flat, calm pancreas doesn't need to secrete much β fat is what fires it up.
