Congenital Heart Defects II. Every defect sorts into one of three buckets by which way the blood moves: RIGHT to left = hypoxia (blue baby), LEFT to right = CHF (heavy fluid), or a valve that is simply stenosed (obstructive). Pick the direction and the whole picture follows.

Blood skips the lungs and goes out unoxygenated — the MOST deadly & MOST tested group.
Orientation: the RIGHT heart is drawn on your left — like you're facing the patient. So forget left/right on the page and watch the colors: 🔵 blue crossing into red = right→left · 🔴 red crossing into blue = left→right.

T - Trouble! — the RIGHT to left blood-flow defects:
⭐ TOF & TGA are the 2 MOST tested. The whole group is the MOST deadly — low oxygen is always a priority on the NCLEX.
The great vessels have traded places: the aorta leaves the RIGHT ventricle and the pulmonary artery leaves the LEFT ventricle. Blue blood is launched straight back OUT to the body.
One single trunk leaves the heart where two separate vessels belong — so lung blood and body blood come out of the same pipe. Another T that steals oxygen.
The tricuspid valve has no opening, so blood can't take the normal road from RA → RV. It detours across to the LEFT side instead — right to left again.
Hypoxia (low O₂) comes from RIGHT to left blood flow — there is decreased blood flow to the lungs, so it results in oxygen problems.
Report Hemoglobin level OVER 22 g/dL — this is PRIORITY & must be reported to the health care provider!
🩸 Why so high? Polycythemia — the hypoxic body builds extra oxygen carriers, so the Hgb climbs and the blood turns thick & viscous. ⚠️ Blood clot risk!
Blood loops back through the lungs instead of going out — the heart gets overloaded and fails.
These four defects push blood from the LEFT side back to the RIGHT: ASD · VSD · PDA · AVSD.
A hole in the septum between the two atria — red blood leaks back into the right side and takes another lap through the lungs.
A hole in the septum between the two ventricles — the strong left pump squeezes blood straight into the right pump. (This same hole is defect #3 of TOF.)
Patent = still open. A duct still connects the aorta to the pulmonary artery, so body-bound blood is siphoned back toward the lungs.
One large central hole where the atrial and ventricular septa meet — an ASD and a VSD in the same spot. The biggest left→right leak of the four.
No shunt at all — the door out is just too narrow.
Stenosis = a narrowed valve. Two to know: Pulmonary Stenosis & Aortic Stenosis. Nothing crosses the septum here — the blood simply can't get out fast enough through a squeezed doorway.
The pulmonary valve is narrowed — the RIGHT ventricle has to force blood through a squeezed doorway on its way to the lungs.
The aortic valve is narrowed — the LEFT ventricle has to force blood through a squeezed doorway on its way out to the body.
